When individuals with severe mental illness won't come to us, our ACT team goes to them.

Individuals suffering from serious mental illness often stop going to programs designed to help with their treatment or stop taking their medication. This can lead to repeated hospitalizations, which disrupt the individual's life as well as the lives of family and friends.

When those with severe mental illness no longer want to come in for treatment, we have a solution: we go to them with our Assertive Community Treatment team (ACT).

Mobile Inderdisciplinary Team

The Pederson-Krag Center has two ACT teams to provide assertive outreach for those with serious mental illness. Our mobile interdisciplinary team offers a full range of services and will travel to the patient's home or wherever the patient wishes to meet us.

Our trained staff includes Registered Nurses, Social Workers, Case Managers,  Psychiatrists and Nurse Practitioners. Team members usually work individually with most patients to meet the needs for food, clothing, shelter, mental health and medical care. Our goal is simple: provide support for the patient's maximum recovery and enhance the individual's quality of life.

Individualized Service plan

In keeping with the premise of ACT, team members will go to the individual's home to make an assessment instead of requiring that person to come to us.

Once the patient is accepted, an individual service plan will be formulated based on a thorough bio-psycho-social assessment. Treatment may include the following:

  • Individual therapy, including crisis intervention and crisis management
  • Substance abuse counseling and treatment
  • Family therapy
  • Medical management and education
  • Illness education, including symptom identification and management
  • Collaboration with other health care providers, including the individual's physician
  • Case management
  • Peer counseling

Serving Huntington to Smithtown

The Pederson-Krag Center operates two of the ten ACT teams throughout the Long Island region and the only teams working with residents from Huntington to Smithtown.
