Clinic Director / Harvey Kasper
Clinic Case Management: Provides advocacy and linkage to
services for anyone (children and adults) in treatment in Huntington,
Smithtown or Wyandanch sites. Clients must be referred by their
treating Therapist or Psychiatrist. New clients can request a referral
for CCM services at time of intake. Case Management not available
to anyone already linked to any other level of case management services.
CCM available 9am - 5pm, M-F by appointment or emergency only.
Clinic Case Manager / Christopher Ferraiolo
- Supervises the Case Manager and Parent Advocate.
- Provides initial concrete services to anyone in treatment, i.e.
- Referrals for housing and food pantries. Referrals for long-term
case management, such as Child Intensive Case Management and Supportive
Case Management.
- Assist clinic clients in determining eligibility and obtaining
benefits such as SSI, SSD, Medicaid, Food Stamps, and Public Assistance.
- Provide clinic clients with an opportunity to register to vote.
- Coordinates special projects such as Children's Holiday Party
and toy drive, Hearts of Hope Fundraiser, Halloween march, Turkey
and food basket drive, and assists in maintaining donations for
thrift shop.
Case Manager
- Provides referrals for Medicaid transportation based on clients'
ability to get to and from their appointments at Huntington, Smithtown
and Wyandanch sites. (Bus tokens available for clients who have
Medicaid but are not eligible for Taxi services.) Referrals for
transportation must be made by the therapist at the client's request.
Assists clients in determining their fee for services. Case Manager
available M-F, 8:30am - 4:30am. Position supervised by the Clinic
Case Manager.
