A Masters graduate from Adelphi University/NYS Licensed, Directing a community based program servicing approximately 288 adult SPMI consumers. Blended Case Management approach ( ICM and SCM services) delivering Assessment & goal planning, Support, Advocacy, ongoing collaboration with the Treatment community, family/support systems, medical community and Social Services as well as Linkages to community resources inclusive of clinical services, housing, financial entitlements, food stamps, medical care, educational opportunities, wellness, chemical dependency programs, food pantries, and social programs.
