The Adult Case Management Program at Pederson-Krag was first established
in 1979 to service the concrete needs of severely mentally ill adults
living in the northwest section of Suffolk County. The goal of the
program is to assist people living with mental illness to attain
and maintain their maximum level of independent functioning and
to avoid inpatient hospitalization.
Case Managers visit clients on a regularly scheduled basis in their
homes. They assess client needs, help them establish goal plans
and link them to community resources. Case Managers routinely advocate
on behalf of their clients and assist them in obtaining services
such as:
- Financial Entitlements
- Housing
- Outpatient Mental Health Care
- Vocational Training
