Blended Case Management provides concrete service to adults age 18
and older diagnosed with a serious mental illness living in the
townships of Huntington and Smithtown. The goal of the program
is to assist these individuals to attain and maintain their
maximum level of independent functioning and coordinate resources to
avoid inpatient hospitalization. Teams consisting of Intensive
Case Managers and Supportive Case Managers visit individuals
on a regularly scheduled basis in their homes and in the community. They assess
client needs, help them to establish individual goal plan and link them to
community resources and eligible benefits. Case Managers routinely advocate on behalf
of their clients and assist them in obtaining service such as:

• Financial Entitlements
• Housing
• Outpatient Mental Health Care
• Vocational Placement and Training
