The Supported Housing Program provides 27 individuals who are seriously and persistently mentally ill (SPMI) with appropriate, affordable, permanent housing as well as on-site support services based on need.

Eligibility requirements:

• Must be over 18 years of age
• Have a primary psychiatric diagnosis (axis I)
• Be capable of independent living

Services include case management and linkage to community resources for vocational/rehabilitation program to optimize the independence and social functioning of severely and persistently mentally ill adults.
